Slate Physical Therapy is an out of network provider, therefore I do not accept insurance. Payments will be collected at time of service. You will be provided with the necessary documentation, a "Superbill", to provide to your insurance company for reimbursement.
Before your visit, I recommend you call your insurance company and ask the following:
What “out-of-network” benefits do I have?
Do you require a doctor referral prior to reimbursement?
How can I submit out-of-network PT documentation (a “Superbill”) for reimbursement?
Payments are accepted via cash, Venmo, check, Credit card, or FSA/HSA.
I commonly inform my patients that when using insurance, you will be paying co-pays which can range on average from $30-$75 per session. If you are going to 1-2 sessions (30 minutes) per week, that amount adds up! I would love to be able to provide you with 1:1 individualized care that is focused on you without the limitations that insurance puts on us. I recognized that it may feel like a big investment to work with me, and I would love to discuss this further with you and answer any questions that you may have!

What outpatient out of network physical therapy benefits do I have?
(They sometimes refer to these as rehabilitation benefits)
What percentage of coverage am I responsible for paying if I see an out of
network provider"?
Does my policy require a written referral?
How do I submit an out of network PT claim (a "Superbill" for reimbursement?
Because I am a sole provider and doing my best to get patients in, please provide 24 hour notice if you need to cancel your appointment. For cancellations that are made outside of the 24 hour window, there will be a fee. No-show appointments will be charged the full price of the service. I fully understand that things come up, and sometimes things are out of our control, this is why special situations will be reviewed on an individual basis.
All of your services will be provided at Slate Physical Therapy, which is located at 667A Saw Mill Brook Parkway, Newton, MA. There is a private bathroom within the clinic, a small gym/fitness area, a treatment table, and a splinting area for hand and upper extremity needs. I strive to make the clinic an inviting space that feels warm, and welcoming from the minute you walk in the front door! There is plenty of free parking available in the parking lot in front of Slate Physical Therapy.
Your first visit with me will consist of a thorough history, and body assessment followed by hands on treatment, education, and exercises to get you started on your journey.
Please wear fitness attire or loose fitting clothing to make assessment easier. There is a bathroom in the clinic if you need to change.
Your follow up visits with me will start with a check in to see how things are feeling and going for you. The remainder of the session will generally consist of manual therapy bodywork, exercises, education, and strategies for independent management at home. I want all of your sessions to feel individualized to you and your specific needs/goals. Generally, people book follow up sessions 1x/week, but if more are needed or desired, this is something we can discuss.
